OunClinic · Nutrition clinic

OunFodmap: the FODMAP diet, step by step, on every patient’s phone

OunClinic is a nutrition clinic. Its nutritionist guides patients with digestive complaints through the FODMAP diet, a three-phase protocol with 222 foods, amounts in grams and rest days between tests. I built an app so that each patient knows what is due today, and a dashboard so that she can see how each one is doing without waiting for the appointment.

The starting point

Where it started

Before

  • A long protocol that is easy to give up on: elimination, reintroduction with 15 tests measured in grams, and final personalisation.
  • The nutritionist wanted to see the records between appointments, not only on the day of the visit.
  • Patients who speak Spanish and patients who speak English.
  • Health data, with up to a thousand patients on the protocol at the same time: protection and GDPR compliance from day one.

The challenge

Turning a complex clinical protocol into something a person can follow alone on their phone, without the nutritionist losing control of any decision.

What was done

Piece by piece, nothing left half-done

Patient app

  • An installable web app for Android and iPhone, without going through app stores, in Spanish and English.
  • A diary for the first phase with amounts and symptoms, Bristol stool scale included.
  • A list of 222 foods that colours itself from what the patient notes: green, yellow, red, or grey if they do not eat it.
  • A warning if they overdo the fruit, a sample menu and guidelines the nutritionist edits from her dashboard.

Automatic reintroduction

  • The nutritionist decides for each patient which groups are tested, in what order and with which food.
  • The app builds the calendar: three test days and four washout days. If a test is repeated, it reschedules everything that comes after.
  • Every morning it reminds them what is due: “today you are on day 2 of fructose: your amount is one and a half tablespoons of honey”.
  • If discomfort appears, it stops the test and sets the threshold at the last amount that went well.

Final phase and reports

  • A FODMAP composition table for 70 foods, with the scientific source of each value.
  • Each patient’s threshold in grams of FODMAP, and amounts of other foods from the same group always below 95% of what they tolerate.
  • A full report and a one-page report for school or hospital, in Spanish or English.
  • No report reaches the patient without the nutritionist’s approval.

Nutritionist’s dashboard

  • She sees the records in real time, without waiting for the appointment.
  • She reviews doubtful foods with one click and decides whether they are ruled out or their threshold is worked out.
  • She gets a notification when a patient marks a food red or finishes a phase.
  • New patients, appointments, notes, and export or deletion of a patient’s data when they ask for it.

Security for health data

  • Consent recorded with the date and the version of the text.
  • Two-factor authentication on the dashboard, protection against brute-force attacks and a log of who signs in and what they touch.
  • Encrypted backups every day, with their restore procedure, and a configurable retention period.

Connected to its AI diet generator

  • OunClinic also has an AI weekly meal plan generator, also built by Meridiano: it writes the plan in the nutritionist’s format and style, with recipes, a shopping list and the allergens for each day.
  • The shopping list and the allergens are calculated by the code, not by the AI: that way they always add up.
  • When the protocol ends, the app passes it each patient’s tolerance profile so that the menu comes out already adapted.

How it works inside

A list that colours itself

The patient does not fill in tables: they note what they eat and how it sat with them. From that, each food on their list takes the colour of the worst reaction they had to it, and grey marks what they never eat.

The app keeps count of what is left to try and, when it is done, tells both of them to book an appointment. The nutritionist sees everything live from her dashboard and decides, with one click, what happens with each doubtful food.

Illustration of the food list with sample data, not from any patient. In the real app, the colour of each food comes only from what the patient notes in their diary.

Composition table, thresholds in grams of FODMAP and reports in two languages.

  5. Delivery 4

    Security and server

Encrypted backups, two-factor authentication, activity log and the app running on the server with a certificate.

Where it is now

The status, no make-up

In use at the clinic

OunClinic already uses it with its patients. It is too early to give usage figures and I am not going to make them up: when there is data on how many patients follow it and how the consultations have changed, it will appear here with its date.
